Consent for Coaching & Peer Support

Purpose
This consent form outlines the nature of coaching services and peer support services provided and ensures the client understands that these services are not to be interpreted as therapy or mental health counseling.
Coaching or Peer Support vs. Therapy
By signing this agreement, I acknowledge and understand that I am receiving coaching (not therapy) or Peer Support (not therapy). I understand the distinction between coaching or peer support and therapy. While therapy is a process designed to treat mental health disorders and emotional difficulties, coaching focuses on personal growth, goal-setting, and actionable steps toward improving specific areas of life, such as career, relationships, and general well-being. Similarly, Peer Support is also different from coaching and therapy. Peer support is a collaborative and empathetic relationship in which individuals with shared life experiences provide emotional, social, and practical support to one another. It’s based on the principle of mutuality, where peers help each other navigate challenges, build resilience, and work toward personal growth. Peer supporters often have lived experiences in areas such as mental health challenges, recovery from addiction, grief, or life transitions, which enables them to connect deeply with those they support.
I understand that:
Coaching is not therapy or counseling, and the coach is not functioning as a licensed therapist or mental health professional. Coaching is goal-oriented and future-focused.
Coaching services are meant to support personal development and are not intended to diagnose, treat, or prevent any mental health conditions.
Peer Support is not therapy or counseling or coaching and a peer supporter is not functioning as a licensed therapist or coach or mental health professional. Peer supporters are trained but typically not licensed professionals. Peer support is a collaborative and empathetic relationship in which individuals with shared life experiences provide emotional, social, and practical support to one another.
Therapy addresses issues related to mental health, trauma, or significant emotional difficulties. Therapists hold advanced degrees (e.g., MSW, PhD, PsyD) and are licensed professionals.
Limits of Coaching and Peer Support
I understand that coaching and peer support:
Will not address mental health concerns such as anxiety, depression, or other psychological disorders.
Coaching is a collaborative, client-driven process focused on setting and achieving goals.
Peer support is a way to address life’s challenges through connection and mutual understanding with a trained individual who can share their lived experiences to provide emotional, social and practical support. While it shares commonalities with therapy and coaching, its unique emphasis on shared experiences and mutual empowerment makes it a distinct and valuable approach for fostering personal growth and emotional resilience.
Coaching and Peer Support Is not a substitute for medical or psychiatric care, and I agree to seek appropriate care for any mental health concerns from a licensed mental health professional.
Each method has its place, depending on the individual’s needs and goals.
Confidentiality
All sessions will be confidential unless I express an intent to harm myself or others, or if required by law.
Acknowledgment and Agreement
By digitally signing below, I acknowledge that I have read and understand the differences between coaching and therapy. I voluntarily agree to engage in coaching services and understand that it is my responsibility to seek out licensed therapy if needed.
This consent form ensures the client fully understands that they are engaging in coaching or peer support rather than therapy and clarifies the boundaries of the service provided. The client also fully understands that Skyler Health is not responsible for any adverse impact, perceived or real, to your physical or mental health or emotional well-being, after your sessions with our coaches or peer support.
